When a Request For Change is submitted, Change Record is created. A change record contains complete information about the change. It is created for every Request For Change that is received, even those that are subsequently rejected. It contains details around type of change, priority, urgency , impact, configuration items impacted, approval/rejection, overall status of the change lifecycle, etc.
